Can I Skip a Dose of Lexapro to Drink? What Happens Next

Published

Key Takeaways
  • Skipping a dose of Lexapro before drinking does not clear it. Its half-life is 27 to 32 hours.
  • By our estimate from the label, more than half of your usual pre-dose level is still in your body a day after a skipped dose, and more if you're 65 or older.
  • The FDA label says to take Lexapro exactly as prescribed, and its Medication Guide says not to drink alcohol during treatment.
  • No study has tested one skipped Lexapro dose, but multi-day gaps are common and stopping can cause discontinuation symptoms.
  • Ask your prescriber about the drink, not the pill. Never double up to make up a missed dose.

No. Skipping a dose of Lexapro (escitalopram) before drinking does not clear it from your body. Its half-life is 27 to 32 hours, so by our estimate more than half of your usual pre-dose level is still there a day later. Skipping just adds a falling drug level to the alcohol. Talk to your prescriber about the drink instead.

The FDA label for Lexapro never offers skipping as an option. It tells you to "Take Lexapro exactly as prescribed," and its patient guide says "Do not drink alcohol during treatment with Lexapro."

Should I Skip Lexapro If I'm Drinking?

It's a common plan: skip the pill Saturday morning, have a few drinks that night, take it again Sunday. It feels like you're keeping the two apart. The label doesn't see it that way.

The same Lexapro label gives advice about alcohol in three places, and each one is a little stricter:

  1. The drug interaction studies, in the clinical pharmacology section, say "Lexapro did not potentiate the cognitive and motor effects of alcohol in a clinical trial. As with other psychotropic medications, the use of alcohol by patients taking Lexapro is not recommended."
  2. The advice doctors are told to give patients notes the experiments were in "normal subjects," but adds that "the concomitant use of Lexapro and alcohol in depressed patients is not advised."
  3. The Medication Guide that comes with your pill just says "Do not drink alcohol during treatment with Lexapro."

So in healthy volunteers, Lexapro did not make alcohol's effects on thinking and movement worse. The label still says not to drink. One line names people being treated for depression, and the Medication Guide says it to everyone taking Lexapro. MedlinePlus adds that "Alcohol can make the side effects of escitalopram worse" and tells you to talk to your doctor about alcohol.

None of these alcohol warnings mention skipping a dose. The question to settle is the drink, not the pill. Bring these to your prescriber:

  • Is any alcohol OK for me right now, and how much?
  • Do my other medicines change that answer?
  • Would a different dose time fit my evenings better?
  • What should I do if I already missed a dose?

Can I Skip Lexapro One Day to Drink? What's Still in Your Body

Lexapro leaves your body slowly. The label lists a half-life of "about 27-32 hours." That's how long it takes for half of it to clear. Taken daily, it builds up: in young healthy adults, steady-state blood levels ran 2.2 to 2.5 times what one dose gives. Steady state takes about a week.

So one skipped day doesn't empty the tank. We did the math from the label's half-life. It's an estimate, not a blood test:

WhoHalf-life (FDA label)Still in your body 24 hours after the skipped dose was due (estimate)
Adults under 6527 to 32 hoursAbout 54% to 59% of your usual low point
Adults 65 and olderAbout 50% longer (roughly 40 to 48 hours)About 66% to 71% of your usual low point

"Usual low point" means the level just before your next dose on a normal day. The label notes the half-life runs about 50% longer in people 65 and older, so even more stays on board.

You aren't drinking without Lexapro. On the night you drink, most of it is still there, and the level keeps sliding until your next pill.

Is a Lexapro Weekend Holiday Safe?

A "drug holiday" means pausing on purpose for a day or a weekend. Lexapro's label warns about stopping, "particularly when abrupt." It lists "dysphoric mood, irritability, agitation, dizziness," electric-shock feelings, "anxiety," "headache," and "insomnia" among the discontinuation reactions.

Does one skipped dose do that? We couldn't find any study that tested a single skipped Lexapro dose. Here is what has been tested:

  • In a 1998 trial of 242 people in remission from depression, placebo replaced sertraline, paroxetine, or fluoxetine for 5 to 8 days. People on the two faster-clearing drugs had more symptoms than those on long-lasting fluoxetine, and more depression symptoms came back.
  • Symptoms can start fast. A 2000 trial found paroxetine symptoms rose "following the second missed dose." The authors tied the timing to how quickly each drug clears.
  • For Lexapro itself, five studies with 1,051 people on escitalopram found stopping it caused more symptoms than placebo, but fewer than stopping paroxetine or venlafaxine XR.
  • And gaps add up. A study that tracked escitalopram pill bottles electronically counted any stretch of 3 or more days in a row with no dose. People had about three of these gaps on average, and the average one lasted 7 days. The study didn't record why doses were missed.

So the research doesn't show that one skip triggers withdrawal. It does show that multi-day gaps are common, and each missed day lowers your level further. For what a longer gap feels like, see our Lexapro withdrawal timeline and what happens when you miss an antidepressant. Antidepressants, escitalopram included, are on our list of medications you should never skip. If you've been skipping on weekends, tell your prescriber.

Is Anxiety After Drinking on Lexapro From the Alcohol, a Missed Dose, or Depression?

A rough day after drinking can come from more than one place, and each needs a different fix:

  • The alcohol itself. NIAAA lists Lexapro with the depression medicines where mixing can bring "increased feelings of depression or hopelessness," in its guide to alcohol and medicines.
  • Missed doses. Anxiety, irritability, dizziness, and poor sleep are on the label's list of reactions to stopping. In the SSRI trials above, symptoms showed up over days of missed doses, faster with quicker-clearing drugs.
  • Depression coming back. Over time, problem drinking can raise the risk of depression. A review in Addiction found that having either alcohol use disorder or major depression roughly doubled the odds of having the other. The authors judged that alcohol use disorder most likely raises depression risk, rather than the reverse.

Tell your prescriber which of these lined up with your bad days. If you ever have thoughts of harming yourself, call or text 988 to reach the 988 Suicide & Crisis Lifeline, as NIMH advises. In an emergency, call 911.

Should I Take My Lexapro Before or After Drinking?

The label has no before-or-after rule. It says to take Lexapro "1 time each day, in the morning or the evening," and "exactly as prescribed." So keep taking it at your usual prescribed time, and don't move it around a night out on your own.

If your evening dose keeps landing near nights out, ask your prescriber whether a morning time makes sense. Here's how a Lexapro time switch is usually done.

Already skipped or forgot? MedlinePlus says to take a missed dose when you remember unless it's almost time for the next one, and never double up. That rule is for forgotten doses, not planned skips. Our missed Lexapro dose guide walks through it. On bupropion too? Its alcohol rules are different. See Wellbutrin and alcohol.

Frequently Asked Questions

Does skipping Lexapro make drinking safer?

No. Lexapro's half-life is 27 to 32 hours, so by our estimate more than half of your usual pre-dose level is still there a day after a skipped dose. If you're 65 or older, the label says the half-life runs about 50% longer, so even more remains. The FDA label does not suggest skipping. It says to take Lexapro exactly as prescribed and not to drink during treatment. Ask your prescriber about alcohol instead.

Is it better to skip the morning dose and drink at night?

It only looks that way. Skipping in the morning still leaves most of the drug in your body that evening, because Lexapro clears slowly. It just lets your level keep falling below its usual low point. Keep your usual time and talk to your prescriber about drinking.

I already skipped my Lexapro and drank. What now?

Don't take two doses to catch up. MedlinePlus says to take a missed dose when you remember unless the next one is almost due, and never to double up. If you've been drinking, call your pharmacist and ask whether to take the missed dose now or wait for your next one. If you feel very unwell or have thoughts of harming yourself, get help right away.

Should I stop taking Lexapro if I want to drink?

Don't stop on your own. The FDA label advises patients "not to abruptly discontinue Lexapro" and to discuss any tapering with their healthcare provider. If drinking matters to you, say so to your prescriber and decide together.

Can I skip Lexapro while I'm still raising my dose?

The FDA label says Lexapro takes about a week of daily doses to reach a steady level. Skipping during that stretch interrupts the build-up. Our first week on Lexapro guide covers what to expect. Ask your prescriber before drinking while your dose is still changing.
