Both pills want an empty stomach in the morning. The Wegovy pill is taken "on an empty stomach in the morning," with a wait of at least 30 minutes before "taking other oral medications." The Synthroid label says to take levothyroxine "on an empty stomach, one-half to one hour before breakfast."

Wegovy Pill and Levothyroxine Interaction: What the Labels Say

The Wegovy label also asks prescribers to "Consider increased clinical or laboratory monitoring for medications that have a narrow therapeutic index or that require clinical monitoring." The Rybelsus label says much the same. Only Wegovy's adds the broader "Monitor the effects" sentence.
What the 33% Levothyroxine Increase Actually Measured
The number comes from one 2021 study run by Novo Nordisk, the drug's maker. It involved 45 healthy adults, not people with thyroid disease. They took a single 600 mcg dose of levothyroxine alone and again while on oral semaglutide 14 mg, the top Rybelsus dose.
"A 33% increase in total T4 exposure was observed with levothyroxine/oral semaglutide vs levothyroxine alone." T4 is the thyroid hormone that levothyroxine replaces, and exposure means the total amount in your blood over time. The peak level did not change. The tablet's absorption helper, called SNAC, "did not affect total T4 exposure" when given alone in the study.
We found no study of levothyroxine with the Wegovy pill's own 25 mg dose. The Wegovy label also notes that with the weekly 1 mg injection, "semaglutide did not affect the absorption of orally administered medications." Those were different trials at a different dose.
Wegovy Pill or Levothyroxine First?
No FDA label for either drug gives an order.
The 2021 study authors wrote that "Levothyroxine has similar dosing conditions to oral semaglutide." A 2026 study from Romania followed 70 people with type 2 diabetes and hypothyroidism who took both drugs, with oral semaglutide at 14 mg. Levothyroxine was given "at least 30 min after oral Semaglutide administration, following current clinical recommendations." The authors did not name the recommendation in that sentence.
That is one research team's approach, not a label rule. Stacking both waits also pushes breakfast later, so settle the order with your prescriber or pharmacist. Consult your doctor or pharmacist for advice specific to your medications.
Will the Wegovy Pill Change My Thyroid Levels?
It might. The 2021 study authors concluded: "Monitoring of thyroid parameters should be considered when treating patients with both oral semaglutide and levothyroxine."
The Synthroid label says that for adults on a stable dose, doctors should check in "every 6 to 12 months and whenever there is a change in the patient’s clinical status." After a levothyroxine dose change, it says to check TSH "after an interval of 6 to 8 weeks after any change in dosage." Starting a new medicine is a fair moment to ask when your next TSH test should be.
In the 2026 study, median TSH in the combined-treatment group fell from 3.1 to 2.6 over six months. The difference from the levothyroxine-only group "was not statistically significant." It was observational, so it can't prove cause.
Is the Wegovy Thyroid Cancer Warning About Hypothyroidism?
Not directly. The boxed warning is about thyroid C-cell tumors, including medullary thyroid carcinoma (MTC). They were seen in rodents; the Wegovy label says it is unknown whether Wegovy causes them in humans. Wegovy is contraindicated (not to be used) in patients "with a personal or family history of MTC or in patients with Multiple Endocrine Neoplasia syndrome type 2 (MEN 2)." Hypothyroidism itself is not named.
If you take levothyroxine after thyroid surgery or cancer, tell your prescriber the exact diagnosis. The Medication Guide also says: "Tell your healthcare provider if you get a lump or swelling in your neck, hoarseness, trouble swallowing, or shortness of breath."
