If you missed a dose of topiramate tablets (Topamax), take it as soon as you remember. If your next dose is less than 6 hours away, skip the missed one and take the next dose on time. Do not double up. Missed more than one dose? Call your prescriber.
That rule comes from the Topamax label and Medication Guide, and the Eprontia liquid label uses it too. The once-a-day capsules, Trokendi XR and Qudexy XR, say something different. So first, check which topiramate is in your bag.

What to Do If You Miss a Dose of Topiramate
The Topamax Medication Guide gives three steps. Take a single missed dose as soon as you can. If you are "within 6 hours of taking your next scheduled dose," wait, take your usual dose then, and skip the missed one. And: "Do not double your dose."
Here is the part people get backwards: the 6 hours count back from your next dose, not forward from the one you missed. Some websites flip it. If you are not sure which time counts as your next dose, ask your pharmacist.
Say you take topiramate at 8 a.m. and 8 p.m. (your times may differ):
| You remember the 8 a.m. dose at | Hours until the 8 p.m. dose | What the Topamax label says |
|---|---|---|
| 11 a.m. | 9 | Take it now |
| 1:30 p.m. | 6 and a half | Take it now |
| 3 p.m. | 5 | Skip it, take the 8 p.m. dose as usual |
| 7 p.m. | 1 | Skip it, take the 8 p.m. dose as usual |
Why not just take two? In a 2015 computer simulation of topiramate levels, doubling a dose pushed the peak level up by 26% to 28%. The Topamax label shows more thinking and memory side effects at higher doses than at lower ones.
Is the Missed Dose Rule Different for Trokendi XR, Qudexy XR or Generic ER?
Yes. Neither extended-release label uses the 6-hour rule. Here is what each Medication Guide says:
| What your label says | How often | If you miss a dose, the Medication Guide says |
|---|---|---|
| Topamax tablets or sprinkle capsules, generic topiramate tablets | Usually twice a day | Take it when you remember, unless your next dose is within 6 hours. Then skip it. Do not double. Missed more than one: call. |
| Eprontia (liquid) | Usually twice a day | Same 6-hour rule. Missed more than 1 dose: call. |
| Qudexy XR | Once a day | Take it as soon as you can. Missed more than one dose: call. |
| Trokendi XR | Once a day | "Talk to your health care provider on what you should do if you miss a dose." |
| Generic "topiramate extended-release" capsules | Once a day | Depends on the maker. Some copy Qudexy XR's wording, others copy Trokendi XR's. |
| Qsymia (phentermine and topiramate) | Once a day, in the morning | Wait until the next morning. Do not double. |

We read six generic extended-release capsule labels on DailyMed. Three use the Trokendi XR line (like Teva's) and three use the Qudexy XR line (like Glenmark's). Go by the Medication Guide that came with this bottle, and check again if a refill comes from a different maker.
Trokendi XR's label does mention "6 hours," but that is its alcohol rule (no alcohol within 6 hours before or after a dose), not a missed-dose window.
MedlinePlus adds a step for all extended-release topiramate that neither brand label has: skip the missed dose if the next one is almost due. If you take an extended-release capsule and remember close to your next dose, ask your pharmacist before taking two that close together.
What Happens If You Miss Two Doses of Topamax (or a Whole Day)?
The Topamax and Qudexy XR Medication Guides say the same thing here: "If you have missed more than one dose, you should call your healthcare provider for advice."
A 2022 simulation study tested catch-up options after one or two missed doses. Its authors wrote that "these results were merely based on simulations," to be used alongside the doctor's judgment on seizure control.
A gap of several days raises a harder question. Topiramate is not started at full strength. MedlinePlus says your doctor will probably "start you on a low dose" and raise it "not more than once every week."
In the Topamax label's migraine trials, "most adverse reactions occurred more frequently during the titration period" (the step-up weeks). It also says: "Rapid titration rate and higher initial dose were associated with higher incidences of cognitive-related dysfunction," meaning thinking and memory problems. So jumping straight back to your full dose after days off is a fair thing to ask about.
None of the topiramate labels say whether to restart at your full dose or step back up after several missed days. Compare lamotrigine: its label tells prescribers to go back to the starting doses after a long enough break (more in our lamotrigine missed dose guide). Topiramate's label leaves the restart to your prescriber, and for the first step-up it already allows slower steps: "If required, longer intervals between dose adjustments can be used."
Questions worth asking when you call:
- "I missed [this many] doses over [this many] days. Should I restart at my usual dose or step back up?"
- "I was still in the step-up weeks. Do I repeat the week I was on?"
- "Which side effects should I watch for while I get back on track?"
- "I take it for seizures. What should I do if I have one before I'm back on schedule?"
Have the count ready: "four doses since Saturday" is easier to act on than "a few."
How Long Does Topiramate Stay in Your System After a Missed Dose?
Long enough that one missed dose is a dip, not a drop to zero. The Topamax label gives a half-life of 21 hours, the time it takes your body to clear half of the drug. The extended-release versions last longer: about 31 hours for Trokendi XR and an "effective half-life" of about 56 hours for Qudexy XR.
The same simulation study sized the dip: skipping one dose lowered the lowest level between doses by about 21% for twice-daily tablets and 27% for Trokendi XR. The dip was bigger in people also taking certain other seizure medicines that speed up how fast topiramate is cleared. It is a computer model, not a blood test, and one of its two authors worked for Trokendi XR's maker. The authors still note that dips like this "may have clinical consequences" for seizure medicines whose effect tracks their blood level.
Can You Stop Taking Topiramate Suddenly If It's for Migraines?
No. Missing a dose and stopping are different things, and the label gives stopping its own warning. Section 5.8 of the Topamax label says: "In patients with or without a history of seizures or epilepsy, antiepileptic drugs, including TOPAMAX, should be gradually withdrawn to minimize the potential for seizures or increased seizure frequency."
The warning is not only for people with epilepsy. MedlinePlus says it plainly: "If you suddenly stop taking topiramate, you may have severe seizures, even if you have not had seizures in the past." The Qsymia label, for a weight-management capsule that contains topiramate, agrees: "Abrupt withdrawal of topiramate has been associated with seizures in individuals without a history of seizures or epilepsy."
There is a migraine reason too. In the 2007 PROMPT trial, 514 people took topiramate for six months, then half switched to a placebo (a dummy pill). The placebo group gained about 1.2 migraine days per 4 weeks. The group that stayed on topiramate gained about 0.1. Some benefit lasted, but the migraine days crept back.
If you want to quit, ask for a plan. The Topamax Medication Guide says your healthcare provider "will tell you how to stop taking TOPAMAX slowly." See whether you can stop a medication when you feel better. For other seizure medicines, see medications you should never skip and our Keppra missed dose guide.
Side Effects After a Missed Topiramate Dose? It May Not Be the Gap
- A migraine on a missed-dose day may just be a migraine. Topiramate lowers how often attacks come, not all of them. In one trial in the Topamax label, people started with about 5.5 migraines every 4 weeks, and the recommended-dose group had 2.1 fewer (placebo: 0.8 fewer). For the attack itself, see whether you can take a second sumatriptan or how many Excedrin you can take.
- Tingling, a foggy head or trouble finding words after you restart can be the medicine itself. Tingling (paresthesia) and memory trouble are common side effects in the label's migraine trials, and its warnings name "word-finding difficulties." But the Topamax Medication Guide says to call your healthcare provider right away for unexplained tiredness, vomiting, slower thinking or reaction time, or changes in mental activities (possible signs of high blood ammonia).
- Overheating on a hot day can come from topiramate's effect on sweating, a label warning, missed dose or not. Call your healthcare provider right away for a high fever, a fever that does not go away, or decreased sweating. More in topiramate and hot weather.
If you take it for seizures and have one, do not wait it out. Get medical help right away and tell your prescriber.
How Pillo Helps
Missed doses are common. In a 2015 study of 8,688 US adults with chronic migraine, only 26% to 29% had enough preventive pills on hand for at least 80% of their days at six months.
Topamax is usually taken as a morning dose and an evening dose: two chances a day to forget. Pillo can hold two fixed alarms a day, and its persistent alarm keeps going until you deal with it.
Its dose history (Adherence Reporting) tracks which doses you took and which you missed, which is what that phone call is about. Refill reminders warn you before the bottle runs low. That way a pharmacy delay does not turn into a sudden stop (see running out of medication before your refill). Managing a teen's topiramate? Dependents Management is free for up to two dependents.
FAQ
I forgot to take my topiramate last night. Should I take it this morning?
For twice-daily tablets, count the hours to your next dose. On an 8 p.m. and 8 a.m. schedule, waking at 6 a.m. leaves 2 hours, so skip the missed one and take the 8 a.m. dose as usual. Remembering at 11 p.m. leaves 9 hours, so take it then. For Trokendi XR, Qudexy XR or a generic ER capsule, follow that bottle's leaflet.
Is it OK to miss one dose of topiramate?
The Topamax label has a plan for exactly that: take it when you remember unless your next dose is within 6 hours (extended-release labels differ). In a computer simulation, one missed twice-daily dose cut the lowest level by about 21%, more for people on certain other seizure medicines. That does not make missed doses harmless, especially for seizures. If it keeps happening, tell your prescriber. To compare other medicines, see what to do if you missed a dose of medication.
How many hours apart should I take topiramate?
The Topamax label splits the daily amount into a morning and an evening dose. The NHS says to try to take each dose at the same time in the morning and evening. Trokendi XR and Qudexy XR are once a day. See what "twice a day" means on a label.
Does the 6-hour rule apply to Qsymia?
No. Qsymia combines phentermine with extended-release topiramate and has its own label. Its Medication Guide says: "If you miss a dose of QSYMIA, wait until the next morning to take your usual dose of QSYMIA. Do not double your dose."
