Three things at once. A smaller stomach. Fewer of the cells that make acid. And acid-reducing medication, which is routinely prescribed after these operations.
The label already flags that last one on its own. It warns that "sucralfate, antacids and proton pump inhibitors may cause hypochlorhydria, affect intragastric pH, and reduce levothyroxine absorption." Surgery and a post-op PPI push the same lever from two directions.
So the question is not how much body is left to dose. It is whether the tablet can still dissolve.
That framing echoes a wider shift. The medical definition of obesity was rewritten in 2025, and BMI was reclassified as a screening number rather than a diagnosis, for much the same reason: one figure was carrying more weight than it could hold.
The surgery you had matters more than the weight you lost
Researchers at a bariatric service reviewed 887 referred patients and analysed the 57 with hypothyroidism, publishing the results in Cureus in 2022. They split them by procedure type and compared against people who did not have surgery.

Two things to read out of that table.
The bypass group needed the most. Not the least. Weight loss did not translate into needing less medication for them.
And the sleeve group came out statistically indistinguishable from people who never had surgery (p=0.67). Same weight loss goal, different answer.
These are group averages from 57 people, only 16 of whom had a bypass, so they describe a pattern rather than a prediction about any one person. The authors' own takeaway is about monitoring, not dosing: "hypothyroid bariatric patients requiring levothyroxine must have their thyroid function monitored regularly."
One more detail worth keeping. In this group, nobody stopped levothyroxine altogether, which ran against earlier literature reporting a 13% discontinuation rate. For these 57 people, surgery did not remove the need for the medication.
Why averages can miss you
The sleeve column above says "no significant difference," and that is true of the group. It is not a promise about individuals.
The 2025 case report describes a 47-year-old woman who developed stubborn hypothyroidism a year after a sleeve gastrectomy. Her doctors ran a T4 absorption test, which measures how much hormone actually gets into the blood after a dose. Her free T4 rose 11% at 120 minutes. The normal threshold is 54%.
That result mattered because it separated two explanations that look identical from the outside. It was not that she was forgetting her pills. The pills were not getting in.
The paper is careful about how unusual this is: "most reports showed unchanged or decreased doses of LT4 needed by patients after sleeve gastrectomy." This case was the exception, and it was resolved by changing the route the hormone was given rather than by simply prescribing more tablets. The recheck schedule is already written down

The label still asks for levothyroxine "on an empty stomach, one-half to one hour before breakfast," and still asks for at least four hours between it and anything known to interfere. The full timing rules do not change just because your anatomy did.
Diet is the other moving piece. The label's interference list names soybean flour, cottonseed meal, walnuts, and dietary fiber as things that can bind levothyroxine and reduce absorption. Post-surgical eating plans often lean on protein supplements and fiber, so a food that was never in your routine before can quietly land in the same hour as your tablet.
