On Zepbound, Wegovy or Ozempic, a 2025 GLP-1 nutrition advisory cites a proposed 1.2 to 1.6 grams of protein per kilogram of body weight a day while you are losing weight, or a flat 80 to 120 grams a day. The catch: the same advisory says using your full current weight can overestimate what you need.

The advisory comes from four groups: the American College of Lifestyle Medicine, the American Society for Nutrition, the Obesity Medicine Association and The Obesity Society (Mozaffarian et al., 2025). For comparison, the everyday recommended amount for adults is 0.8 grams per kilogram. None of the drug labels set a protein number. The Zepbound prescribing information simply says the drug "decreases calorie intake" and that the effect is "likely mediated by affecting appetite." Less appetite is why protein gets hard.

How Much Protein a Day on Zepbound?

There is no Zepbound-specific protein number. The advisory covers GLP-1 medications used for weight loss and gives reference points, not one rule. Per-pound figures are our conversion (1 kg = 2.2 lb).

With kidney disease, skip the higher numbers until you talk to your doctor. The NIDDK notes that some people with chronic kidney disease may need moderate amounts of protein so waste does not build up in the blood.

Should You Use Your Current Weight or Your Goal Weight?

This is the part many online protein calculators skip. They take your weight, multiply it by 1.2 to 1.6, and hand you a number. The advisory says it is unclear which weight the math should use: your actual weight, an adjusted or ideal weight, or your fat-free mass. It also says "the use of actual weight can significantly overestimate protein requirements" for people with obesity (Mozaffarian et al., 2025).

A calculator that multiplies your full weight counts every pound the same, fat or muscle. The advisory mentions two options, with no consensus yet:

  1. Base it on lean mass instead. About 1.5 grams per kilogram of lean body mass is considered more accurate, but you need a body composition scan to know your lean mass.
  2. Use the flat 80 to 120 grams. No math, and it may be easier to stick to.

Which one fits you is a question for your prescriber or a dietitian.

How Much Muscle Do You Lose on Zepbound?

Some, but most of what you lose is fat. Both the Zepbound and Wegovy labels say the drug "lowers body weight with greater fat mass loss than lean mass loss" (Zepbound label, Wegovy label). Body scans from the two big trials put numbers on it.

  • In SURMOUNT-1, the Zepbound trial, 160 people had body scans. Those on tirzepatide lost 21.3% of their body weight over 72 weeks: about 35 pounds of fat and 12 pounds of lean mass. That is roughly 75% fat and 25% lean. The placebo group, who lost much less weight, lost it in the same 75/25 split (Look et al., 2025, a Lilly-funded analysis).
  • STEP 1, the Wegovy trial, scanned 140 people. Those on semaglutide lost about 18 pounds of fat and 12 pounds of lean mass over 68 weeks (Wilding et al., 2021, supplementary table S5). The advisory puts that at 62% fat and 38% lean. Even so, the share of the body made up of lean mass went up, because fat dropped faster.

Two cautions before you read that table. These were different trials with different people, scanners and analysis methods, so this is not a head-to-head comparison, and it does not show that one drug protects muscle better. And "lean mass" is not the same as muscle. It includes water, organs and everything else that is not fat or bone. The advisory estimates muscle is about half of lean mass, which works out to roughly 20% of the weight lost in STEP 1 being muscle.

How Much Protein per Meal on Zepbound?

Spread it out instead of saving it for dinner. A small study tested this in 8 healthy adults. Eating about 30 grams of protein at each of three meals led to 25% more muscle protein building (called synthesis) over 24 hours than eating the same total mostly at dinner (Mamerow et al., 2014). It was not in GLP-1 users, and it measured muscle building, not muscle size.

Doing the math on the advisory's flat target, 80 to 120 grams split over three meals is roughly 27 to 40 grams each. Split over four smaller eating times, it is 20 to 30 grams. The advisory also suggests eating the protein part of the meal first, before you get full (Mozaffarian et al., 2025).

The advisory calls fish, eggs, Greek yogurt, cottage cheese and nut butters "lower volume, nutrient-dense" choices, which helps when a few bites fill you up.

Protein Shakes on Zepbound: Do You Need Them?

Not necessarily. The advisory says some people meet their targets with high-protein shakes, bars and fortified foods (Mozaffarian et al., 2025). On a low-appetite day a shake may be easier to get down than a plate of food.

How Much Protein and Fiber to Avoid Constipation on Zepbound?

The advisory does not name a protein amount that prevents constipation. It warns that protein-heavy foods can make it worse. Constipation showed up in 11% to 17% of people on Zepbound in its main trials, compared with 5% on placebo (Zepbound label).

The advisory explains that foods high in protein, fat or fiber slow your stomach down even more. That "can also worsen constipation," sometimes enough that people need to cut back on those foods for a while (Mozaffarian et al., 2025). The same slowed stomach is behind other side effects like sulfur burps. If constipation lasts, tell your prescriber before you push protein higher.

How to Prevent Muscle Loss on Zepbound

Protein is only half of it. The advisory is blunt: more protein alone "is likely inadequate to support the preservation of muscle mass in the absence of structured resistance/strength training" (Mozaffarian et al., 2025). Strength training, or strength plus cardio, is well established to help preserve lean mass during weight loss. Cardio alone has a smaller effect.

The advisory flags some people as more likely to lose muscle when appetite drops. They include older adults, women around or after menopause, people with lower testosterone, and people who sit most of the day or do no strength work. If that is you, bring up both protein and a strength plan at your next visit.

Feeling Weak on Zepbound? It May Not Be Low Protein

Other things on these drugs can cause weakness or tiredness too.

  • The drug itself. Fatigue was reported in 5% to 7% of people on Zepbound versus 3% on placebo in its main trials (Zepbound label).
  • Dehydration, since nausea, vomiting and diarrhea can drain fluids, which can lead to kidney problems. The label says: "Tell your healthcare provider right away if you have nausea, vomiting, or diarrhea that does not go away."
  • Low blood sugar, mainly if you also take insulin or a sulfonylurea. Weakness, shakiness, hunger and dizziness are on the label's list of warning signs, along with a fast heartbeat (we cover heart rate on these drugs separately). Tell your prescriber if you notice them.

One more label instruction worth knowing: "Stop using ZEPBOUND and call your healthcare provider right away if you have severe pain in your stomach area (abdomen) that will not go away, with or without nausea or vomiting." The Wegovy label has the same instruction for Wegovy.

Modeling cited in the advisory suggests muscle makes up about 10% to 15% of weight lost in women versus 20% to 25% in men without strength training.
