Paxlovid and Statins, BP Pills and Eliquis: What Changes?

Published
October 19, 2026
Key Takeaways
  • Paxlovid contains ritonavir, which slows the enzyme your body uses to clear many statins and other heart medicines. Some need a pause, a lower dose or extra monitoring.
  • The FDA label says to stop simvastatin and lovastatin at least 12 hours before, during, and for 5 days after Paxlovid. For atorvastatin, it says to consider a temporary pause.
  • Calcium channel blockers like amlodipine may need a lower dose. The label says to avoid Xarelto, Plavix and Brilinta with Paxlovid, and the Eliquis plan depends on your dose.
  • Restart days differ by drug and by source, from the day after to 5 days after the last Paxlovid dose. Ask your prescriber for each medicine's restart date.
  • Don't stop or change a heart medicine for Paxlovid on your own. Your prescriber or pharmacist decides whether each one needs a dose change, a pause or extra monitoring.

Paxlovid contains ritonavir, which slows the enzyme your body uses to clear many statins and other heart medicines. The FDA label tells prescribers to stop simvastatin and lovastatin at least 12 hours before, during, and for 5 days after. Your prescriber may also pause atorvastatin, lower amlodipine or Eliquis, and avoid Xarelto. Lisinopril, losartan and metoprolol aren't named in the label.

Your prescriber or pharmacist decides whether each medicine needs "a dose adjustment, interruption, and/or additional monitoring," in the label's words. So don't stop or change a heart medicine for Paxlovid on your own.

Call your prescriber or get medical help right away if you have:

  • Unusual bleeding or bruising, bleeding you can't stop, red or black stools, or red, pink or brown urine (Eliquis Medication Guide)
  • Muscle pain or weakness you can't explain (Lipitor Medication Guide)
  • Fainting, severe dizziness, or a heartbeat that feels slow

Call 911 for trouble breathing, chest pain or pressure that won't go away, new confusion, trouble staying awake, or pale, gray or blue lips, nail beds or skin (CDC).

The Paxlovid leaflet says to stop taking Paxlovid and get medical help right away for allergic reaction signs, like hives, swelling of the face, lips or tongue, or trouble swallowing or breathing.

Why Does Paxlovid Interact With So Many Heart Medications?

Paxlovid is two drugs in one pack. Nirmatrelvir fights the virus. Ritonavir keeps it in your blood longer by blocking an enzyme called CYP3A4. That enzyme also breaks down many statins, calcium channel blockers and blood thinners. When that enzyme is blocked, some of your usual pills can build up. Among serious reactions reported in the label, the most common co-medications were transplant drugs like tacrolimus, "followed by calcium channel blockers" like amlodipine.

The block builds, peaking about 48 hours after the first dose. And it lingers: ritonavir permanently disables the enzyme, so your body has to make new enzyme, which takes several days.

Paxlovid and Statins: Which Ones Get Held?

It depends on which statin, and atorvastatin and rosuvastatin get different treatment. "Held" means your prescriber may tell you to pause it.

What about a top Google result, "Statin Needs to be Continued During Paxlovid Therapy"? It's a 2022 letter to the editor, an opinion, not a guideline. It agrees simvastatin can't be combined. It argues high-risk people shouldn't lose statin protection, so it suggests fluvastatin or pravastatin "to substitute for simvastatin."

So the real question is narrow: does a few days without any statin matter? Liverpool says a short pause "will not negatively affect the therapeutic effect." The letter says statin therapy "should not be discontinued." Neither cites outcome data for a 5 to 10 day gap, so it's your prescriber's call.

Paxlovid and Blood Pressure Medication

Calcium channel blockers (amlodipine, diltiazem, verapamil, nifedipine, felodipine). The Paxlovid label says "caution is warranted" and "a dose decrease may be needed." Liverpool's amlodipine page predicts about 2 times more amlodipine in your blood. It says that usually means a lower dose, but a change can be optional if your prescriber gives you a plan. That plan covers what low blood pressure signs to watch for and when to pause. For diltiazem and verapamil, the JACC review adds a slow heartbeat. Same enzyme story as grapefruit and blood pressure medication.

Most other blood pressure pills. The Paxlovid label doesn't name lisinopril, losartan, metoprolol or hydrochlorothiazide. Liverpool rates lisinopril, metoprolol and hydrochlorothiazide "no interaction expected," and losartan a "potential weak interaction" with no dose change advised up front. Exceptions include valsartan, Entresto (sacubitril/valsartan), doxazosin and terazosin, which Liverpool rates a "potential interaction," with a watch for low blood pressure. The label says to avoid aliskiren (Tekturna), and eplerenone and ivabradine are on its do-not-combine list.

Liverpool also warns that if a drug isn't on its list, you can't assume it's safe to combine. Online checkers help you build your list, but your prescriber makes the call.

Can Paxlovid Raise or Lower Your Blood Pressure?

Both show up. The label lists "hypertension" among reactions reported after Paxlovid's emergency-use rollout, and its patient leaflet lists "high blood pressure" as a possible side effect, with no rate given.

Low blood pressure is the risk when Paxlovid raises a blood pressure drug, mostly calcium channel blockers. Amlodipine has a half-life of about 30 to 50 hours. The block starts fast and peaks around 48 hours, and Liverpool expects its effect on amlodipine to last up to 3 days after the last dose. So a drop can come any time during the course or just after. Liverpool's entry gives no specific day.

If your prescriber asks you to check your blood pressure at home, write down each reading. Lightheaded when you stand, or well below your normal? Call before your next dose.

Can You Take Paxlovid With Blood Thinners Like Eliquis?

Blood thinnerUS Paxlovid labelWhat your prescriber may do
Apixaban (Eliquis)Bleeding risk goes up. The plan depends on your doseLower the dose, or avoid the combination if you're already on the lowest dose (Eliquis label)
Rivaroxaban (Xarelto)"Increased bleeding risk." Avoid togetherPick another COVID treatment, or another blood thinner for a short time
Dabigatran (Pradaxa)Lower the dose or avoid, depending on use and kidneysAdjust based on kidney function
Warfarin"Closely monitor" INRExtra INR checks, especially right after
Clopidogrel (Plavix)Avoid. Paxlovid lowers its active formDepends on how recent your stent is
Ticagrelor (Brilinta)Avoid. Paxlovid raises itSwitch antiplatelets or pick another COVID treatment

The biggest rule is in the Eliquis Medication Guide: "Do not stop taking ELIQUIS without talking to the healthcare provider who prescribes it for you. Stopping ELIQUIS increases your risk of having a stroke." The Xarelto guide says the same: don't stop "unless your doctor tells you to." Here's what one missed blood thinner dose means for stroke risk.

Paxlovid can make Plavix weaker, so the worry is a clot, not bleeding. IDSA's 2022 guide says to avoid Paxlovid for 6 weeks after a coronary stent. The JACC review says within 6 months, or after a high-risk procedure, unless your doctor can switch you to prasugrel, which some people can't take. Tell whoever prescribes Paxlovid your stent date.

Paxlovid Medications to Avoid, and Why Some Heart Patients Are Told No

The label's do-not-combine list includes amiodarone, dronedarone, flecainide, propafenone, quinidine, ranolazine, eplerenone, ivabradine and finerenone. Simvastatin and lovastatin are on it too, but the label says they "can be temporarily discontinued to allow PAXLOVID use."

Amiodarone shows why. The JACC review notes its half-life of about 100 days "precludes the option of temporarily stopping the drug."

When Paxlovid isn't a fit, the CDC lists remdesivir as the second preferred treatment, then molnupiravir, which works less well and is not for use in pregnancy. Did a pharmacist send you back to your doctor? That's the system working. The FDA checklist tells pharmacists to refer you when "modification of other medications is needed due to a potential drug interaction."

When to Restart Statins and Other Meds After Paxlovid

Sources disagree most here. Liverpool's modeling says over 80% of the block has faded 3 days after the last dose in most people. Some people are slower.

MedicineRestart timing, by source
Simvastatin, lovastatinUS label: 5 days after the last dose. Liverpool: at least 3, preferably 5. JACC: 5
AtorvastatinUS label: no need to hold after. Liverpool and JACC: 3 days after, if paused
RosuvastatinJACC: the day after. Liverpool's general rule: 3 days after
Amlodipine, Eliquis, XareltoLiverpool: back to the usual plan 3 days after
Statins in general (IDSA, 2022)Hold "for 5 days after"

The answer depends on the drug and your prescriber's reference. Before your first dose, ask:

  • Which of my medicines do I pause, and starting when?
  • Which ones change dose, and to what?
  • What symptoms mean I should call you?
  • What calendar date do I restart each one, and at what dose?

If you ever need a second Paxlovid course, ask for a new plan. IDSA says the length of treatment should guide changes to your other medicines.

Are Muscle Aches on Paxlovid From Your Statin or From COVID?

COVID itself causes muscle or body aches and tiredness. So aches on day 2 aren't automatically your statin.

But a statin that builds up can hurt your muscles, and Lipitor's label tells prescribers to stop it if muscle disease is suspected. The Lipitor Medication Guide says to report "unexplained muscle pain, tenderness, or weakness, especially if you also have a fever or feel more tired than usual." COVID brings fever and tiredness on its own, so you can't sort this out at home. Call.

In one published case, a 72-year-old on atorvastatin had worse muscle aches on day 2 of Paxlovid. His team stopped Paxlovid, held the statin and ran a blood test for muscle damage. It was normal, and he restarted the statin a week after finishing Paxlovid. The authors wrote that the best approach "may have been" a full Paxlovid course with the statin held. That's one person, not a rule.

How Pillo Helps

A 5-day Paxlovid course can mean one pill paused, another at a new dose, and restart dates that differ by drug.

In Pillo, add a dose note to each changed medicine, like "paused for Paxlovid, restart Oct 27 per my doctor." Set the restart day as a reminder, and the persistent alarm keeps going until you act. If your prescriber asked you to watch your blood pressure, log readings in the blood pressure and heart rate trackers. Managing a parent's heart medicines? Dependents Management, part of Pillo's paid plan, keeps their schedule in your app.

Frequently Asked Questions

Can you take Paxlovid with rosuvastatin (Crestor)?

There's no single rule. The US label gives no rosuvastatin instruction. Liverpool advises pausing it during treatment, and the JACC review suggests holding it and restarting the day after. Your prescriber picks the plan.

Can you take Paxlovid with amlodipine?

The label allows it with caution. The Paxlovid label says "a dose decrease may be needed" for calcium channel blockers like amlodipine. Liverpool predicts about 2 times more amlodipine in your blood and says the usual dose returns 3 days after the last Paxlovid dose if it was changed. Ask your prescriber whether to change it and what low blood pressure signs to watch for.

Can you take Paxlovid with Eliquis?

Sometimes, at a lower dose. The Eliquis label says to cut the dose when it's taken with ritonavir and similar drugs, and to avoid the combination if you're already on the lowest dose. Liverpool says the usual Eliquis plan resumes 3 days after the last Paxlovid dose. Never change an Eliquis dose on your own.

Can you take Paxlovid with metoprolol?

The US Paxlovid label doesn't name metoprolol, and Liverpool rates it "no interaction expected." The JACC review lists it as safe to co-administer. Keep taking it unless your prescriber says otherwise.

Do I need to stop aspirin for Paxlovid?

Aspirin isn't named in the US Paxlovid label, and Liverpool rates aspirin taken as a blood thinner "no interaction expected." In the JACC review's example, a heart patient was told to keep taking his. Either way, don't stop heart aspirin without asking the prescriber who started it.

Does Paxlovid affect your heart rate?

It can, through other drugs. Ivabradine is on the label's do-not-combine list for slow heart rate risk, and the label calls for digoxin blood level checks. The JACC review flags a slow heartbeat with diltiazem and verapamil.
