Picking a cold medicine for high blood pressure starts with two ingredient types: oral decongestants (pseudoephedrine, phenylephrine) and NSAID pain relievers (ibuprofen, naproxen). Their labels, like Sudafed's, say to ask a doctor before use if you have high blood pressure. "HBP" cold products skip the decongestant. But several add an antihistamine the American Geriatrics Society lists as one to avoid at 65 and older.
That last part is easy to miss, because the HBP box looks like the safe pick.

Call 911 if your blood pressure is higher than 180/120 and you have symptoms such as chest pain, shortness of breath, back pain, numbness, weakness, a change in vision or difficulty speaking. That's the American Heart Association's rule: "Do not wait to see whether your blood pressure comes down on its own."
Decongestants and Cold Medicine to Avoid With High Blood Pressure
Check the Drug Facts label for these ingredients:
| Ingredient | Found in | What the label says |
|---|---|---|
| Pseudoephedrine (decongestant pill) | Sudafed, Advil Cold & Sinus, Aleve-D | Ask a doctor before use if you have high blood pressure, heart disease, diabetes, thyroid disease or trouble urinating due to an enlarged prostate |
| Phenylephrine (decongestant pill or liquid) | Sudafed PE, DayQuil Cold & Flu, Alka-Seltzer Cold | The same "ask a doctor" list |
| Oxymetazoline (nasal spray) | Afrin | The same list, plus do not use for more than 3 days |
| Ibuprofen, naproxen (NSAIDs) | Advil Cold & Sinus, Aleve-D | Ask a doctor if you have high blood pressure, heart or kidney disease, or take a diuretic (water pill) |
| Chlorpheniramine, doxylamine (drowsy antihistamines) | Coricidin HBP Cold & Flu, NyQuil HBP | Ask a doctor if you have glaucoma or trouble urinating due to an enlarged prostate |
Sources: labels for Sudafed, Sudafed PE, DayQuil Cold & Flu, Afrin, Advil Cold & Sinus, Aleve-D, Coricidin HBP Cold & Flu.
The American Heart Association lists decongestants and NSAIDs among drugs that can raise blood pressure. Don't count on your own pill to warn you: the Zestril and Cozaar labels each have an NSAID section. The lisinopril (Zestril) and losartan (Cozaar) labels do not mention decongestants.
Can I Take Sudafed With High Blood Pressure Medication?
Sudafed's active ingredient is pseudoephedrine. The Sudafed label says to ask a doctor before use if you have high blood pressure. It also says not to use it with a prescription MAOI, or for 2 weeks after stopping one.
So how much does it raise blood pressure? A 2005 review of 24 trials found pseudoephedrine raised the top number (systolic) by about 1 mmHg on average. It had no effect on the bottom number. The authors added: "Higher doses and immediate-release preparations were associated with greater BP increases."
Can I take Sudafed with controlled high blood pressure?
In the same review, people with controlled high blood pressure saw a similar average rise in the top number: 1.20 mmHg. That's an average, not a prediction for your arm cuff. A 2025 Cochrane review of oral decongestants taken daily for a week or more found little to no effect on the top number. But only one of its five trials tested pseudoephedrine, the volunteers averaged 20 years old, and it rated that evidence "very low-certainty."
The Sudafed label doesn't split controlled from uncontrolled. The prescription Clarinex-D 12 Hour label, which contains pseudoephedrine, goes further: it "should not be used in patients with severe hypertension or severe coronary artery disease." Ask your doctor whether a decongestant fits your readings.
Is Phenylephrine Safe for High Blood Pressure?
Phenylephrine is the decongestant in Sudafed PE, DayQuil Cold & Flu and Alka-Seltzer Cold. In November 2024, the FDA proposed removing oral phenylephrine after its review found it "is not effective" as a nasal decongestant. The FDA said this is "based on effectiveness concerns, not on safety concerns." Its consumer page says it "identified very few cases of serious side effects" when oral phenylephrine is taken as directed.
As of October 21, 2026, FDA has not issued a final order on oral phenylephrine; the November 2024 order is still a proposal. So they can still be sold.
The catch: Sudafed PE and DayQuil Cold & Flu labels still say ask a doctor first if you have high blood pressure. So you'd take on that warning for an ingredient the FDA says doesn't work as a pill. The FDA says nasal spray phenylephrine isn't part of the proposal.
Fizzy tablets add sodium, which the AHA says to check for: each Alka-Seltzer Cold Cold & Headache tablet lists 356 mg.
Can You Take Advil Cold and Sinus With High Blood Pressure Medication?
Advil Cold & Sinus and Aleve-D pair a decongestant with an NSAID. Their labels say ask a doctor first if you have high blood pressure, heart or kidney disease, "or had a stroke," or if "you are taking a diuretic." Both carry this warning: "NSAIDs, except aspirin, increase the risk of heart attack, heart failure, and stroke. These can be fatal." Our Tylenol and naproxen guide lists who else those NSAID lines cover.
Your blood pressure pill's own label adds more:
| Your blood pressure medicine | Example | What its label says about NSAIDs |
|---|---|---|
| ACE inhibitor | Lisinopril (Zestril) | NSAIDs may weaken its blood pressure effect. In people who are older, low on fluids (including from a diuretic) or have weak kidneys, the pair "may result in deterioration of renal function, including possible acute renal failure," usually reversible |
| ARB | Losartan (Cozaar) | The same two warnings |
| Thiazide diuretic | Hydrochlorothiazide | In some people, NSAIDs can reduce its effect, so you should be "observed closely" |
| Beta-blocker | Propranolol (Inderal LA) | NSAIDs "have been reported to blunt" its blood pressure effect |
Sources: labels for Zestril, Cozaar, hydrochlorothiazide, Inderal LA.
In a 2013 BMJ study of 487,372 people on blood pressure drugs, an NSAID with just one of these (a diuretic, an ACE inhibitor or an ARB) was not linked to more kidney injury. A diuretic plus an ACE inhibitor or ARB plus an NSAID was: 1.31 times the rate seen without the NSAID, and 1.82 times in "the first 30 days of use." Pills like Zestoretic (lisinopril with hydrochlorothiazide) already combine two of the three.

Afrin Nasal Spray and High Blood Pressure
A spray isn't a free pass. The Afrin label (oxymetazoline) has the same "ask a doctor" line for high blood pressure. It also says not to use it for more than 3 days, because frequent use "may cause nasal congestion to recur or worsen."
Saline is one swap. In 2019, AHA News reported that blood pressure guidelines suggested "nasal saline or antihistamines" in place of decongestants.
Coricidin HBP Cold and Flu Ingredients and Side Effects
The Coricidin HBP box says "Decongestant-free." Coricidin HBP Cold & Flu has no decongestant: its two active ingredients are acetaminophen and chlorpheniramine.
Chlorpheniramine is an older, drowsy antihistamine. The American Geriatrics Society's 2023 Beers Criteria list first-generation antihistamines, including chlorpheniramine and doxylamine, as medicines to avoid in adults 65 and older. They're "highly anticholinergic," with a "risk of confusion, dry mouth, constipation." The Coricidin HBP Cold & Flu label itself says "drowsiness may occur."
HBP products differ. Coricidin HBP Nighttime and Vicks NyQuil HBP use doxylamine, which is on the same Beers list. Coricidin HBP Chest Congestion & Cough lists dextromethorphan and guaifenesin as its active ingredients, and DayQuil HBP lists acetaminophen and dextromethorphan. If you're 65 or older, ask your pharmacist which one fits.
Decongestants and an enlarged prostate
Men with prostate trouble should read both kinds of labels. Sudafed, Sudafed PE, Afrin, Advil Cold & Sinus, Coricidin HBP Cold & Flu and NyQuil HBP all say ask a doctor before use if you have trouble urinating due to an enlarged prostate gland. The Beers Criteria add that strongly anticholinergic drugs "may decrease urinary flow and cause urinary retention" in men with these symptoms.
What Cold Medicine Can I Take With High Blood Pressure Medication?
Pick a product for the symptom you have. Here's what these labels say to check:
| Symptom | Ingredient | What to check first |
|---|---|---|
| Fever, aches, sore throat | Acetaminophen (Tylenol) | Liver warning: more than 4,000 mg in 24 hours. Ask first if you have liver disease or take warfarin |
| Dry cough | Dextromethorphan (Delsym) | Not with a prescription MAOI or for 2 weeks after stopping one. Ask first about a chronic cough |
| Chest congestion | Guaifenesin plus dextromethorphan (Mucinex DM) | Same MAOI line. Ask first about a chronic cough or lots of phlegm |
| Stuffy nose | Saline spray | Suggested in place of decongestants (AHA News, 2019) |
| Runny nose, sneezing | Antihistamine | See the 65-and-older note above |
Sources: labels for Tylenol Extra Strength, Delsym, Mucinex DM; AHA News.
The Tylenol Extra Strength, Delsym and Mucinex DM labels do not list high blood pressure as a reason to ask a doctor first. But one brand can cover very different boxes: DayQuil Cold & Flu adds phenylephrine. And the FDA notes acetaminophen "is in many common brands of nonprescription drugs." The Tylenol Extra Strength and DayQuil Cold & Flu labels say not to use them with any other drug containing acetaminophen. Tylenol with NyQuil and DayQuil then NyQuil walk through that overlap, and took too much Tylenol covers what to do if you went over. Your pharmacist can check any box against your medicine list before you buy.
Blood Pressure High After Taking Sudafed? What to Do
Here's what the labels and the AHA say to do:
- Know the stop signs. Sudafed says stop and ask a doctor if you get nervous, dizzy or sleepless. Advil Cold & Sinus adds signs of heart problems or stroke: chest pain, trouble breathing, weakness in one part or side of the body, slurred speech or leg swelling. Don't just call the office for those. MedlinePlus says to call 911 if you think you're having a heart attack or a stroke.
- Recheck the right way. If your reading is higher than 180/120, the AHA says wait at least 1 minute and take it again. Still that high, with any symptom in the 911 box above? Call 911. Still that high without symptoms, and you're not pregnant? Contact your health care professional "as soon as possible."
- Been taking it for days? Sudafed says stop and ask a doctor if symptoms "do not improve within 7 days or occur with a fever." Afrin's limit is 3 days.
- Don't stop your blood pressure pill on your own over a cold or a high reading. The AHA says, "Do not stop taking prescribed medications without talking to your healthcare professional."

What Cold Medicine Can I Take With AFib or After a Heart Attack?
If you have AFib or a past heart attack, the labels add a few specifics:
- Pseudoephedrine "can produce" effects such as insomnia, dizziness, tremor or "arrhythmias" in some patients, says the Clarinex-D label. Sudafed says ask first if you have heart disease.
- On a blood thinner? NSAID cold labels like Advil Cold & Sinus say the chance of severe stomach bleeding is higher if you "take a blood thinning (anticoagulant)" drug. Acetaminophen labels say ask first if you take warfarin.
- If you take aspirin for heart attack or stroke, Advil Cold & Sinus and Aleve-D say ask first, because ibuprofen or naproxen "may decrease this benefit of aspirin."
- On a beta-blocker like propranolol or metoprolol, note that the Clarinex-D label says pseudoephedrine may reduce beta-blockers' blood pressure effect. The over-the-counter Sudafed, Advil Cold & Sinus and Aleve-D labels do not mention beta-blockers. Advil Cold & Sinus and Aleve-D do say to ask a doctor or pharmacist first if you are "taking any other drug."
Timing matters too. A Taiwan study of 9,793 heart attack patients linked a respiratory infection in the prior week, without NSAIDs, to about 2.7 times the odds of a heart attack. With NSAIDs during the infection, it was about 3.4 times. With AFib or a heart condition, ask your cardiologist or pharmacist before any cold product.
Can a Cold Raise Your Blood Pressure?
The big studies here tracked heart attacks after infections, not blood pressure readings. In a 2018 Ontario study, heart attack hospital admissions ran about 6 times higher in the 7 days after a lab-confirmed flu than in the year before and the year after. For RSV the ratio was 3.51, and for other viruses 2.77. To see what a cold does to your own numbers, measure at home. If you've been diagnosed with high blood pressure, the AHA says to "track your blood pressure readings and medications."
How Pillo Helps When You're Sick
Pillo's free Health Trackers include blood pressure, heart rate and body temperature. Free dose spacing alerts flag two doses scheduled too close together, handy for cold medicines taken every 4 to 6 hours. The alerts work from your schedule, not from ingredient lists, so check boxes for acetaminophen yourself.
And the persistent alarm keeps ringing for your regular blood pressure pill until you deal with it, even on a foggy sick day. Pillo is free for unlimited medications, with ads.
FAQ
What cold medicine can I take with high blood pressure and diabetes?
Decongestant labels like Sudafed and DayQuil Cold & Flu list both high blood pressure and diabetes in their "ask a doctor" line. Liquids may add sugar: Coricidin HBP Nighttime lists high fructose corn syrup. Tell your pharmacist about both conditions.
Can I take Mucinex with high blood pressure?
It depends on which Mucinex. The Mucinex DM label does not list high blood pressure as a reason to ask a doctor first. Other Mucinex boxes, like Fast-Max Severe Congestion & Cough, add phenylephrine. Read the active ingredients on the box you're holding.
Is DayQuil High Blood Pressure different from regular DayQuil?
Yes. Vicks DayQuil and NyQuil High Blood Pressure Cold & Flu have no decongestant. DayQuil Cold & Flu adds phenylephrine, and its label says ask a doctor first if you have high blood pressure. NyQuil HBP still contains doxylamine, an antihistamine on the Beers "avoid" list for adults 65 and older.
What is the best cold medicine for high blood pressure?
There isn't one box that fits everyone. Match the product to your symptom. Skip decongestants and NSAIDs unless your doctor has said yes. If you're 65 or older, check any antihistamine against the Beers Criteria. Single-ingredient products, like plain acetaminophen for fever or dextromethorphan for a dry cough, make that easier to check. Show your pharmacist your blood pressure pills before you buy.
Is Sudafed PE better than Sudafed for high blood pressure?
Not on the label. The Sudafed PE and Sudafed labels carry the same "ask a doctor" line for high blood pressure. The difference is that the FDA has proposed removing oral phenylephrine, the PE ingredient, because its review found it "is not effective" as a decongestant.
Can I take cold medicine with high blood pressure and kidney disease?
NSAID cold products need the most care. Advil Cold & Sinus says ask a doctor first if you have kidney disease. The lisinopril label warns that NSAIDs with ACE inhibitors may worsen kidney function, including possible acute kidney failure, in people with "compromised renal function." Ask your doctor before any cold product.
Can I take Tylenol with blood pressure medicine?
In 2019, AHA News reported that blood pressure guidelines named acetaminophen as an alternative to NSAID pills. Its label warns about the liver and warfarin. It still isn't zero-effect. In the PATH-BP trial, people with high blood pressure took 4 grams a day for 2 weeks, and their daytime top number rose about 5 mmHg compared with placebo. Use only what you need.
